Yes, the cupholders need to come out for the glove compartment (upper part) to come out.
No, I didn't have a CD changer beforehand.
You will need reprogramming if you don't already have a CD changer installed. If you do, I'm really not sure what the procedure is. I know that the 5-er and 6-er guys have been having good luck transplanting the iPod adapter for their CD changer without reflashing...
