That is a question where a lot of variable will need to be accounted for.

For the drag:

1. Speed
2. hp rating
3. Weight of the car

For wind/tire noise:

1. Type of tire and brand
2. Tire pressure
3. Car insulation

But, in general, you will not notice any difference.
having more surface area exposed to the open will create more drag when it's a wheel since they're not exactly aerodynamic I'm curious if you hear any more noise because with most people I see running spacers where they're flush by me I can hear noise pretty easily. I love the look but would need to see for myself I suppose

I don't disagree with you for what it's worth. My X5 has acoustic glass to further reduce the noise
My personal experience, as far as flush look, I do not notice any difference.
The amount of road noise and mpg are the same.
