Above poster is correct you can not get non run flats unless you opt for 22" wheels (at least in the US)
So when making your comparison, try to keep things apples to apples. Same wheel size, same suspension ect. But for noise and comfort, non run flats will always win in the same size and tire model.

You should get the spare wheel regardless of what tire you pick. It's a $150 when placing order or $900+ if you try to add it after taking delivery. In my experience, I have never used the empty space in the spare wheel well and I am so glad I opted for spare wheel in my newer cars.
The only caveat is for the x3, it will eat into your rear cargo space but that's not the case for the x5.
